New Directions strives to support individuals to live life on their own terms. Our person centered planning process assists all stakeholders in understanding the lifestyle desired by the individual.

The lifestyle plan is developed by meetings that bring together individuals, family members, case managers, agency staff, advocates and other concerned parties to gather information, define preferences and propose lifestyle objectives and activities.

Supportive Housing Services:

The goal of New Directions supportive housing services is to afford individuals housing opportunities beyond what has traditionally been available. The mission of supportive housing is to assist individuals in accessing and retaining quality affordable housing with the availability of on-site or accessible supportive services.

In-Home Support Services:

In-Home supports are designed to assist individuals in residing successfully in their own living arrangement. We provide services to individuals of all abilities and ages in their family home, family foster care and their own apartments.

Supported Living Services:

Supported Living Services are provided to individuals in homes of New Directions that have been licensed as adult foster care. Individuals receive services which are funded by either the Home and Community Waiver or the CADI Waiver.

During the past thirty years New Directions has coordinated support and services to individuals of all intellectual abilities including persons with significant physical challenges, mental health issues and autism, individuals who require personal assistance in all areas of basic life maintenance, and individuals who exhibit many excessive behaviors including self-injurious behaviors and physical and environmental aggression. In all of our support services, assistance is offered in meal planning, shopping, budgeting, health monitoring, medication management, social and recreational skills, developing relationships and community connections, and pursuing vocational interests.
